This confounds Yankees, and it’s not hard to see why. It can either mean that you are commiserating with/sympathetic to the person whose heart is being blessed, or it is an example of sympathetic exasperation.
“Bless her heart, that husband of hers is worthless trash”.
“She keeps trying to wear that style of dress, bless her heart”.
